Sending attestation questionnaires
How to monitor responses, chase non-responders, review submissions, and accept or reject questionnaires.
Once you have sent an attestation campaign, this guide covers how to manage it through to completion: monitoring progress, reviewing submissions, and recording your decisions.
Monitoring progress

Open Attestation from the sidebar, then click the Campaigns tab.

Find your campaign in the list. The campaign card shows a progress summary: how many recipients have been sent the questionnaire, how many have opened it, how many have started it, and how many have submitted.

Click the campaign card to open the campaign detail view, which shows the status for each recipient individually.
Chasing non-responders
If a recipient has not submitted by the deadline, you can send them a reminder.

In the campaign detail view, find the recipient with a pending status.

Click Send reminder on their row. This sends them a new email with their original questionnaire link.
The questionnaire link remains valid even after the deadline. Recipients can still submit after it passes, and you can still accept their submission.
Reviewing a submission
When a recipient submits their questionnaire, the status on their row changes to Submitted. You need to review the submission and accept or reject it.

Click Review on the submitted row.

The submission opens, showing each question and the response the recipient gave. File uploads are available to download.

Review each response. If something is unclear or requires evidence you do not have, you can send the questionnaire back with a note asking the recipient to clarify or add more information.
Accepting or rejecting a submission
After reviewing:
- Click Accept if the submission meets your requirements. The recipient's status changes to Accepted and the result is recorded in your attestation log.
- Click Reject if the submission is unsatisfactory. Add a note explaining why. The recipient's status changes to Rejected.
You can change a decision after the fact, for example accepting a submission after the recipient has provided additional evidence.
The attestation log
Accepted and rejected submissions are recorded in your attestation log. This gives you a time-stamped audit trail showing which suppliers have been assessed, what they submitted, when, and what decision was recorded. The log is accessible from the campaign detail view.
After the campaign closes
Completed campaigns (those where all recipients have either submitted or where the deadline has passed) remain in the Campaigns tab with a Completed status. You can re-open them for reference, export the results, or create a follow-up campaign for the same recipients using a new or updated template.
